CEBT Open Enrollment

CEBT Open Enrollment Period:

Begins: Monday, November 3
Ends: Friday, November 14

Our open enrollment period for our new Medical, Dental, and Vision benefits with CEBT will run from November 3rd through November 14th. During this time, you’ll have the opportunity to enroll in your new plan options. This enrollment will be a “positive enrollment” meaning, all benefit eligible employees need to complete the open enrollment process.

If you do not complete the open enrollment process, you will no longer have Medical, Dental, and Vision Insurance coverage effective 01/01/2026.

We have worked closely with CEBT to help educate and guide you through the enrollment process. To support you, we have hosted benefit presentations at the PD building. Benefits Changes (Qualifying Events)

Please contact monica.williams@summitk12.org if you have had a qualifying event and wish to make changes to your benefits outside of the open enrollment period. Examples of qualifying events may include:

New dependent by marriage, birth, or adoption,
Change in hours of employment that affect full-time status,
Dissolution of marriage or domestic partnership,
Dependent reaching the age of 26 unless they have a diagnosed disability,
Employee or dependents become entitled to Medicare or Medicaid benefits,
Death.

Benefit Summaries

Benefit Eligibility:

Employees working 30+ hours per week are eligible for benefits. New employees have 30 days from date of hire to make their elections. All benefits are effective as of the first day of the month following the date of hire.

Life and Accidental Death & Dismemberment Insurance

Life Insurance provides financial security for the people who depend on you. Your beneficiaries will receive a lump sum payment if you die while employed by SSD. The District provides basic life insurance of $25,000 at no cost to benefit-eligible employees, regardless of enrollment. All employees must provide beneficiaries.

Accidental Death and Dismemberment (AD& D) insurance provides payment to you or your beneficiaries if you lose a limb or die in an accident. SSD provides AD&D coverage of $25,000 at no cost to benefit-eligible employees regardless of enrollment. This coverage is in addition to your district-paid life insurance described below.

Accidental Injury Insurance

United Health Care Accidental Injury Insurance to pay you, or whoever you designate, for treatments or injuries resulting from a covered accident. It can help you pay for expenses such as rehabilitation, transportation, child care, travel, or other out of pocket expenses. Coverage continues after your first covered accident and can help provide protection for future covered accidents.

Hospital Care Insurance

Critical Illness Insurance

United Health Care offers Hospital Care Insurance to help you pay for a covered hospital stay. With UHC Hospital Care Insurance, you receive a check for a qualified hospitalization resulting from a covered injury or illness. There are no copays, deductibles, coinsurance, or network requirements.

Short Term Disability Insurance

When trouble arises, Short-Term Disability insurance can provide employees with the peace of mind that a protected paycheck brings. UHC’s Voluntary Short-Term Disability plan provides income if you become disabled due to an injury or illness after satisfying the elimination period (7 days). Once enrolled in the plan, you can receive up to 60% of your weekly earnings or a maximum of $1,500 per week for up to 12 weeks. Visit your employee portal for further plan details.
