Summit School District employees that are benefit eligible (.75 FTE or more, working 30 hours or more weekly) will have the opportunity to enroll into Medical, Dental, Vision, Voluntary Life Insurance, Health Savings Accounts and Flexible Spending Accounts through the CEBT portal. Voluntary Insurance benefits offered through the Standard will be available to enroll in the Easy Apps portal.

Dental, Medical, Vision and District Life

IMPORTANT:

This year’s Open Enrollment is an ACTIVE ENROLLMENT. The insurance elections you make during this period will determine your coverage through the District for the plan year July 1, 2026 – June 30, 2027. If you do not complete open enrollment you and your dependents will lose coverage as of June 30, 2026.. 

NOTE: If you are planning to retire or leaving the District at the end of the school year - You WILL need to go through open enrollment in order to maintain coverage for the summer. ACTION IS REQUIRED. This is important, if you do not complete open enrollment you and your dependents will not be covered over the summer. Your current Insurance plans will terminate as of 6/30/2026. Your current insurance will not carry over because this is an active enrollment. 

If you are planning to decline insurance starting 7/1/2026, you will still need to go in and decline/waive all insurance elections, and verify/update your Beneficiaries for the District Paid Life Insurance policy.

New! SSD is now offering a New Embedded High‑Deductible Health Plan (HDHP4).

Designed to give employees more flexibility based on financial and family considerations our three medical plan options will include:

• PPO5 – PPO5 SBC, 

• HDHP2500 – HDHP25 SBC

•  HDHP4000 - HDHP4 SBC

Deductibles and Out of Pocket Expenses:

• Deductibles and out-of-pocket maximums run on a calendar year basis (January 1 – December 31).

• These amounts reset on January 1, 2027.

• Any deductible or out-of-pocket expenses you have met in 2026 will continue to apply through the remainder of the 2026 calendar year.

Coming Soon: Step-by-step enrollment instructions will be sent this week. Also, WTW will offer 1-on-1 meetings via Calendly. The booking link to meet with them will be posted soon.

Voluntary insurance

Changes: There are a few changes to be aware of this year: SSD will transition voluntary benefits from UnitedHealthcare to The Standard, effective with the new plan year.

• Accident Insurance

• Critical Illness

• Hospital Indemnity

To meet with The Standard, schedule time to meet with a benefits enrollment counselor they will be able to walk you through your voluntary benefit options and help you complete your enrollment with confidence.

CEBT Open Enrollment Presentation

Cards will be mailed to the address on file for each employee. If you have not received your cards or if you need additional cards, you may log into the CEBT community. See CEBT Manage Your Account flyer for more information. You have access to order and download insurance cards in the CEBT portal.

Benefit Eligibility:

Employees working 30+ hours per week are eligible for benefits. New employees have 30 days from date of hire to make their elections. All benefits are effective as of the first day of the month following the date of hire.

2025 Open Enrollment Webinar

Prescriptions are available through CVS/Caremark. You can view, download and order your Caremark card by logging into the Cebt Portal and selecting " ID Card" https://www.cebt.org/cvs-caremark

If you are on any maintenance medications (blood pressure, cholesterol, etc), you can set up Mail Order by calling 866-885-4944 or by setting up your prescription delivery at caremark.com. The 90 day mail order is 2X your regular copay and allows you to get an extra month of your prescription at no additional cost.

ALLONE HEALTH - EMPLOYEE ASSISTANCE PROGRAM (EAP) -Information Flyer

The Allone Health EAP offers 6 free counseling sessions per year for CEBT members and dependents under 26, covering issues like divorce, parenting, grief, and conflict.

It also includes 6 free life coaching sessions, legal review, and financial counseling.

Telehealth benefits are available to all employees through Teledoc. There is NO COST for an office visit, but employees are responsible for any associated treatment costs.

TELADOC HEALTH - TELEMEDICINE
Teladoc gives members 24/7 access to U.S. board-certified doctors through the convenience of phone, video, or mobile app visits for non-emergency illnesses, including flu, allergies, sinus infections, and more. To access:

Dental benefits are through Delta Dental. Please navigate to the website for more information and flyers: https://www.cebt.org/delta-dental

You can view and order cards in your portal.

Vision benefits are through VSP. https://www.cebt.org/vsp

VSP is a national not-for-profit vision care company that provides vision payment and access to their provider network for members that are on the CEBT's Vision B or Vision C plans. There is no card if you have a VSP plan, providers will be able to look you up by name or social security number.
